Mechanism of action
Aztreonam works on the same principle as that of penicillin. By preventing peptidoglycan crosslinking, it prevents the bacterial cell wall from being synthesised. It shows a strong preference for penicillin-binding protein-3 and a weak preference for penicillin-binding protein-1a.
Aztreonam binds poorly to Gram-positive and anaerobic bacteria’s penicillin-binding proteins and most of the time is not effective against them.  Aztreonam is bactericidal in nature.

Pharmacokinetics
Absorption- After following oral dosing, it gets absorbed less than 1 percent from the gastro intestinal tract and following intramuscular administration, it is completely absorbed.
The volume of distribution appears to be 12.6 L

Protein Binding- Serum protein binding is 56 percent on average and dose-independent. Renal dysfunction affects 36 to 43 percent of people.
Metabolism- By hydrolysis of the beta-lactam link, approximately 6 to 16 percent of the molecule is converted to the inactive metabolites, as a result an open-ring compound observed.

Route of elimination- Aztreonam is eliminated in the urine through active tubular secretion and glomerular filtration nearly equally in healthy people. A single parenteral dose was almost completely excreted in the urine 12 hours after injection.
Half Life- Aztreonam’s serum half life was 1.7 hours (1.5 to 2.0) in participants, with normal renal function regardless of dosage. The mean serum half life of aztreonam rose in older patients and those with impaired renal function (4.7 to 6 hours and 2.1 hours, respectively).
Clearance- The clearance has been found 91ml per minute for a healthy person.
